Hospital Aftercare

A spell in hospital can leave you feeling very anxious and sometimes quite vulnerable.  It might be that you need some help to get back into your normal routine.  Or you might have to adapt to a new one.   Our Hospital Aftercare Service provides reassurance and practical help to get you back on your feet.  We will help to build up your confidence and make sure that you are getting all the support you need from other agencies.

If you are an adult who has recently been discharged from hospital and you don't qualify for help through social services, then our service may be for you.  It is available to people living within Gloucestershire County Council's boundaries.

We have a team of trained volunteers who work alongside our staff to provide you with the help you need.  Our volunteers are carefully chosen to bring you the best and most suitable support.

We do not make any charge for this service.

We will help you with daily tasks such as light shopping, answering letters, paying bills, or going for a walk.  You might just want to have a chat over a cup of tea when your volunteer will be able to give you information about other ways in which we might be able to help you.

One of our Hospital Aftercare Co-ordinators will visit you as soon as possible after you are discharged.  Sometimes we are able to see you before you go home if we are given enough notice.

After the initial visit, subsequent visits will be arranged at times that suit you and your volunteers.

Our volunteer will visit you for up to six weeks.  The number of visits each week will depend on the help you need.  Visits normally last for between one and one-and-a-half hours.

How can I arrange to be seen?

We just need to have your details,   You, your doctor, a nurse, hospital social worker or family member can ring with these.   One of our co-ordinators will then get in touch with you and arrange to call.   Just telephone us on:   01452 420907.   Or click below to download a leaflet
